Making Appointments
For your convenience, we recommend appointments be made prior to each visit. Appointments can be made in person or by phone.
Same-Day Appointments
We accept a limited number of same-day appointments. Please keep in mind that your wait time may be longer, since patients with scheduled appointments will be seen first.

Your First Appointment
To prepare for your first visit, your physician must review your previous medical records. Please arrange to have these forwarded to our office from your current health care provider, and mail or bring to us any records you have at home before your first visit. We ask that you arrive at least 20 minutes early for your first visit to complete patient registration. Please bring your insurance card with you.
Because we take the confidential nature of your health information very seriously, we will give you our Notice of Privacy Practices. It describes how we may use your health information and outlines your privacy rights under the Health Insurance Portability and Accountability Act (HIPAA) of 1996, a federal regulation governing patient privacy. We will ask you to sign the Acknowledgment of Privacy Practices form to signal that you have received this notice. Only individuals given permission on the form may access your health information such as test results or copies of records.
What to Bring with You
When visiting the health center for the first time, you will need to bring two forms of proper identification, which may include a social security card, valid driver’s license, birth certificate, or another form of official identification. If available, you should also bring your Medi-Cal card, Medicare card, or private insurance card. If uninsured and applying for the CFMC Discount Program, you will also need to bring a pay stub, an unemployment check stub, a recent tax return, or a letter from your employer in order to prove your family income. |
Health Insurance Programs
We accept Medi-Cal, Medicare, and most private insurances. Our friendly health center staff can get you in touch with Medi-Cal outreach workers and representatives from Healthy Families who can assist patients with enrollment in these programs.
Uninsured
For the uninsured, we accept payment using our Patient Discount Policy, based on family size and family income. Please ask our health center staff about the CFMC Discount Program and eligibility. You will be required to bring your phone or utility bills for proof of California residency.
Co-payment Policy
We appreciate your co-payment at the time of your visit. For your convenience, we accept cash, checks, Debit, VISA, and Mastercard. We will give you a receipt for your payment records.

Family PACT Program
Administered by the Department of Health and Human Services, the Family Planning, Access, Care, Treatment (PACT) program provides clinical services for family planning and reproductive health at no cost to low-income residents, including teenagers. This fills a critical gap in healthcare for the indigent, uninsured, and working poor of the Coastside. |

Who is eligible?
Women and men are eligible for the Family PACT program if they reside in California, are at risk of pregnancy or causing pregnancy, have a gross family income at or below 200 percent of the Federal Poverty Level and have no other source of health care coverage for family planning services.
What benefits are covered?
- Initiation and management of all methods of contraception, including emergency contraception and male/female sterilization
- Clinical and preventive services to maintain reproductive health, such as testing and treatment for sexually transmitted infections, breast, cervical and testicular cancer screening, and periodic exams
- Individual reproductive health education and counseling

Healthy Families
Healthy Families is a state program that provides low-cost health insurance for children in families whose income is below 250 percent of the Federal Poverty Level. With Healthy Families, a family pays a small amount each month to receive health care for their children. |

Who is eligible?
- Children from birth through 18 who are not eligible for no-cost Medi-Cal. Children may not have employer-sponsored insurance.
- Children whose family income is too high to qualify for no-cost Medi-Cal but is within Healthy Families’ income limits (up to 250% FIG).
- Children who have not had any insurance coverage from an employer during the three months before she or he applies to the Healthy Families Program, with some exceptions.
- Your child stays eligible for a continuous 12 months once it is determined that she or he is eligible, regardless of changes in your family income—as long as premiums are paid.
- Children whose families qualify for food stamps or the free-and-reduced lunch program are likely to be eligible for Medi-Cal or Healthy Families.
What benefits are covered?
- This program offers health, dental, and vision services
- The health care benefits are similar to those given to children of CalPers employees
- Enrolled children are eligible for all covered services that are medically necessary—children are not excluded from coverage due to pre-existing conditions
How do I apply?
- Call the Healthy Families Program toll free at 1-888-747-1222
- Visit the Healthy Families website. There, you also can locate a Certified Application Assistor (CAA) in your area to help you with the application
- Ask your physician or the front office staff for an application
- With the help of a CAA, you can submit an on-line application using Health-e-App by visiting www.dhs.ca.gov/health-e-app, where you may also learn more about the benefits.
