Patient Rights & Responsibilities
At Coastside Family Medical Center (CFMC), we recognize that talking about personal health issues is not always easy. We strive to ensure patients are comfortable in informing us of their needs, and in return ask that patients provide us with the information to best address these needs. We have put together a list of Patient Rights & Responsibilities to help guide your next visit.
Corporate Compliance
CFMC is committed to a pro-active approach in complying with federal, California, and local laws in our practices. To meet this commitment and to help our employees understand our expectations, CFMC has set forth a Compliance Program, which governs all of our board members, administrative, and professional personnel in the manner in which CFMC’s business is to be conducted. Please review our Corporate Compliance practices.
Health Insurance Portability & Accountability Act (HIPAA)
As a healthcare provider, CFMC's practices fall under HIPAA, which provides rights and protections for participants and beneficiaries in group health plans. We ask that patients review the HIPAA guidelines to ensure that they are aware of how CFMC may use or disclose your protected health information to carry out treatment, payment, or healthcare operations, or for other purposes that are permitted or required by the law.
Please review our Privacy Practices. We also ask that patients fill out the Acknowledgement of Privacy Practices form after reading the Privacy Practices.
